Hip Pain??

My daughter is having some chronic hip pain and I thought maybe someone on the team had experienced something similar.  The pain is mostly on the side of her hip.  But if she tries to run, or even do extended walking, it wraps around to the front.  She's been to the doctor and PT without much help.  They thought it was muscle related rather than joint related.   Any ideas?  Trigger point?  rollers?  Thanks

  • Bob- you don't say:
    1. how old is she
    2. normal activity level
    3. testing performed
    4. initial cause
    5. treatment tried (besides PT)

If she isn't using foam roller, tennis ball massage, professional massage for a muscle issue- these are all things she can try. But more info would be helpful.
  • Wow, how old is she ?

The Pros' don't know either .... ? No MRI, No X-ray..

If she were an adult the first thing that comes to mind is bursitis. Then perhaps some anti inflammation supplements with easing up of activities.

Get yourself a better team of doctors. Keep us posted whatever you decide and good luck.

  • Did they do a FABER test? It's a simple manipulation of the leg to see if it illicits pain, which would be suggestive of a labral tear.

X-ray to look at the shape of the acetabulum would also be worthwhile, quick, and cheap.

From the muscular side, piriformis syndrome, IT Band syndrome would be worth ruling out, as would the suggestion above of bursitis.

Best advise at this time, if it hurts, DON'T do it.
  •  Hip pain that is in front of the hip or in the groin suggests a joint problem rather than soft tissues.  If conservative measures aren't helping, an MRI and sports medicine consultation are probably the best course.

  • You guys are good. Next time I have an issue I am coming to you!!! I just got over a sore hip that sounded similar which is why I read this post. Never saw anyone. Just stretched my IT band and used foam roller on it. I'm old. I sort of get pains here and there all the time. Hope she is better soon.

  • Thanks for the feedback. To answer the questions above. She is 32. She has been to her GP, and an x-ray was done. She was told to ice the hip, take ibuprofen, and had a couple of appointments at PT. The doctor thought that it mignt be bursitis, but wasn't sure - unclear to me why the doctor couldn't tell. She believes the initial cause was a modified gate when she had a badly sprained ankle and was wearing a "boot" to support the ankle. The ankle is better, but the hip is not. The pain is on the side of the hip, but "wraps around" if she tries to run or take a long walk. She can ride a stationary bike without pain. She is going back to the doctor, and is asking for a referral to an orthopedic specialist. At Kaiser, the GP's don't do MRI's. Hopefully they will figure it out. I also pointed here to Leigh's web site. I've found useful information there before when I had issues.
  • Look up muscular trigger ponts of quadratus lumborum and iliopsoas

+1 for this. IANAD but odds are that if her gait was modified she compensated by changing normal movement patterns--some things tightened, some things loosened, and all of them need to be reset. It also won't help much if she has a sedentary job in front of the computer, which as oft as not pulls you forward and shortens both the hip flexors and everything that attaches to them. For starters, I'd try a hard diet of lunge stretches with arms raised, cobra and pigeon lunge poses, slow squats with no weight but good form (for stretching, not strength), monster walks w/bands-- you get the idea. Or to simplify it even further--get her to move sideways. A lot.
