Posterior Tibial Tendon

I think I have a little case of Posterior Tibial Tendon inflammation.  After long runs the inside of my right ankle is stiff and painful, so much so it is difficult to walk the next day.  No pain when I run, just afterwards. I also have some crazy arthritis thing in the big toe joint of the same foot and the flexibility of this toe is reduced by about 40-50%.  I am sure there is a relationship.  Haven't been to the doc yet.  Not sure if I should see an ortho or a podiatrist or a physiatrist or if should just ice it and do NASAIDs and suck it up.

  • You likely have poor stability proximaly and you are tight likely in hips, calves etc. all the excess hip knee ankle movement from instability makes your ankle and big toe work harder. The longer you run the more flared up. It came on after as inflammation comes on later. My bet is if you kept running on it you would hurt while running and with only 50% great toe motion your whole gait is off. I say stop running. Ice your sore toe and ankle then address the problem which is likely elsewhere. I can't diagnose you without seeing you BUT if I were you I would

    Open up my back- rotations in upper and lower
    Stretch hip flexors and roll ITB and Quads
    Roll calf muscles and do downward dog
    Stretch my great toe into flexing if pain allows


    Don't run in pain

    Then I would strengthen my core, hips, and work on single leg balance
    Planks, side planks
    Single leg balance, walking lunges

    I would go to a smart PT who does not chase your pain but looks for the cause too

    Just icing the toe and ankle and waiting for pain to subside then running again may put you in the same boat
  • Wow, thanks Carrie.  That is a lot of great advice. I definitely have a tight IT Band and will be more diligent about stretching and rolling.
