Severe Lateral (outer) foot pain after bike: Abandoned Run

On my brick yesterday I had a good bike pretty much on target with my plan.  I did cut it from 4 hours to 3:15 so I would not be late to a late afternoon Mothers Day dinner.




At the end of my bike I was tired and when I got off the bike on solid pavement I noted a sharp pain in my right foot.  It is on the lateral side of the midfoot (outside of the foot).  I noted the soreness on both feet but the right foot pain was acute.  So much so that I limped walking and running.  I got about 3/4 of a mile into my run and saw that no good was going to come of the run -- it hurt so much -- that I abandoned the run.  The pain is at the point noted in the following illustration.

This is not the first time I have noted this pain, but it has never been this acute to my memory.  It is a little better today and I can jog around the house without a limp.  I tend to run and ride on the outside of the foot.  I can see this from the wear in my running shoes.  The bike shoes are in their third season and are Shimano TR31's.  I have noted this foot pain before but can't say I have ever abandoned a run.




My foot is very flat and I notice in these Shimano bike shoes that my foot is not supported on the medial side (inner) as much as the outer.  I'm thinking of trying an insert with more arch support.  I have an old custom orthotic from running that I can insert and try to take up the room on the arch to more even the foot load when pedaling.

  • Have you had a bike fit that includes a close inspection of your cleats? A good bike fitter will most likely see something there and shim your cleats. Also with your running shoes are you in Neutral shoes or do they have any stability? I doubt the problem is in your running shoes as the pain was there immediately off the bike. I would take a good look at your bike shoes/cleat placement or better yet have a profesional do it. It would be worth $100 to prevent an injury that could lay you up for weeks.
  • Running shoes are stability shoes (new Saucony Hurricane's).  I've been thinking bike fit and am inquiring in the area (central Arkansas) as to who is the best.

  • Pat- if you are wearing the OUTSIDE of your shoes then you don't want a stability shoe with the inside post. That will roll you more to the outside. Although I think your primary concern needs to be bike shoes and cleats.
