Cortisone Flare, the Extended Version?

Hoping for a WSMd (made that up!) or anyone with relevant experience can shed some light on what's been a very uncomfortable couple of days. Short version: I have a bum shoulder and ended up getting a cortisone shot in it on Thursday morning. Going in, my pain was a 4 -5. For a few hours afterwards, it felt better (due to the lidocaine, I presume) but then the pain increased dramatically (like to a 9 and I don’t use 10) and has stayed there. I can’t lift it at all, can’t sleep, waaaahhh, etc.
I’ve read about Cortisone flare and that made sense right up until about 12 hours ago. Everything I can find says symptoms of that generally subside after 48 hours tops. Well, I’m going on 60 hours with no end in sight. Any ideas? Could it be something else?

I've never had an injection in my shoulder but had it for PF and for metatarsalgia in 4th toe a few years back. It hurt during as well as after for the metatarsalgia as the small joint was overloaded w/ the solution and joint was swollen and painful for a day or two but then it was great and painfree since. I iced it big time right after so I'd recommend that but would also give the doc a call if it is not better by end of today or tomorrow AM at latest. Good luck and feel better.
